Disabled Persons Parking Permit

To apply for a disabled persons parking permit please download the relevant application below and submit in person at one of our Service Centres or fax to (03) 5128 5672 .

Individuals: Please ensure that your Doctor has filled in and signed page two of the forms prior to lodgement.

Please note that individual parking permits are issued for a period of 3 years and organisation parking permits are issued annually.
