Home and Community Care

The Home and Community Care Program aims to provide assistance to frail older people, people with functional disabilities and the carers of these people, so that they may continue to live independently in the community.

The Home and Community Care team delivers a range of services as part of the Home and Community Care (HACC) program.

The HACC programs follow a comprehensive list of guidelines and are funded by the Commonwealth, State and Local Governments.
